Become A Patient

Unfortunately the MCCG is currently ONLY accepting applications for its Designated Grower Program, while our dispensing services have been interrupted.

If your doctor has prescribed medical cannabis for your condition, but is reluctant to sign Health Canada forms, please contact the MCCG. We now have doctors that are willing to take patient referrals and sign Health Canada forms.

Interested in becoming a patient of the Medical Cannabis Club of Guelph?

If you are already a Health Canada Licensee or a patient of another Medical Cannabis Club^© approved dispensary within Canada then you do not need to fill out the application package. Simply proceed to contact us by telephone or email.

Membership with the MCCG can be obtained using one of the 3 forms below.

Form B1 application is for category 1 applicants, meaning the following conditions only:
MS, HIV/AIDS, cancer, epilepsy, spinal injury/disease, severe arthritis

Form B2 application is for category 2 applicants, meaning any condition not listed on Form B1. Note: this form must be signed by a specialist relevant to the condition.

Limited Membership application is for patients in the process of applying for a Health Canada permit. It grants them temporary access to medicine based on the consent of a medical practitioner. It can be used for either category 1 or category 2 applicants (any condition). Unfortunately this option is not currently available.

Usually within 5 business days, your application will have been verified and you will be contacted to setup your first visit to the Medical Cannabis Club of Guelph. At your first visit you will be given your patient identification card so that you can access your medicine when you need it.
